A scanned copy (including signatures) of the roster/waiver form must be sent to registration@torontophoenix.com with the team name in the subject.
The roster/waiver form deadline is August 8, 2014 at 12:01am. Failure to submit the roster/waiver form by the deadline will result in a $25 penalty.
